 in Home Health Care

Quality is measured against the ultimate yardstick - patient outcomes. Outcome reports are powerful tools when utilized in meeting your goal of providing the best possible care. Learning to use these tools will provide direction for continuous quality improvement in your agency. In addition, understanding your outcome reports and how they reflect clinical practice can also make a big difference in how successful you are in today’s competitive market.  

The participants in this workshop will learn to interpret and utilize home health outcome reports (OBQM, OBQI, Case Mix, and Tally Reports) to monitor and improve the quality of patient care. Learn to focus quality and performance improvement activities on select target outcomes, investigate the care processes that contributed to these outcomes, and make agency-specific changes in clinical actions that lead to improved patient outcomes. 

 

OBJECTIVES

• Discuss the purpose of OASIS derived Outcome Reports.

• Interpret OASIS derived Outcome Reports (OBQM, OBQI, Case Mix, and Tally Reports.)

• Select Outcomes to target for improvement.

• Identify at least two Quality Improvement tools and methods.
